◙ MAILINGS WILL BE GOING OUT THIS WEEK (Feb. 23-27) to both you and us. your letter will include a PRE-POPULATED form with information on the BCBS plan you are eligible to choose.  Also, if you have a rider on your LifeWise policy your package will include a BCBS waiver that most closely matches the LW rider.

◙ WE ENCOURAGE YOU TO USE THE PRE-POPULATED FORM from BCBSAZ and send it back to us. You may either fax it to us at 520-318-9400 or 888-301-4329 toll free;  or you may also email us by using our secure email service...  CipherSend 43U82 Secure email   You may fax BCBS @ (602) 864-3151 however we don't recommend it. If you fax BCBS direct please let us know so we can try to track it.

◙ You must send LifeWise your March premium... the first effective date avalible with BCBSAZ is April 1st.  If your paperwork isn't processed by April 1st but is processed in the month of April your effective date will be May 1st.
