The Copay Plan Emphasizes value with no annual
maximum limitation or network waiting periods. This Plan is not a prepaid plan,
but rather a scheduled PPO where members pay copays and providers are reimbursed
fee-for-service.
The PPO Plan Offers flexibility while providing low member coinsurance and competitive
premiums. With The PPO Plan, members will enjoy significant savings by selecting
from a network of providers or by opting for a non-network provider and still
receiving rich benefits.
The Indemnity Plan Provides freedom of choice in selecting dentists. Under this plan members may
select any dentist. Within a family each person may choose their favorite
dentist or the most convenient one.
